Hello to all our friends in Surat Thani, Koh Samui, and throughout the South! I am "Dr. Teng," Dr. Apirak Wongsawasup, the resident Plastic Surgeon at Aurora Clinic.

Today, I ask permission to take off my clinic owner's hat and sit down with you as "a friend who happens to be a doctor"—a friend who won't lie to you, a friend who will tell you the truths you might never have known, and a friend who wants to see you smile at the mirror again.


Have you ever noticed? Some days you wake up feeling refreshed and mentally energetic, but as soon as you step out, a friend asks, "Didn't you sleep last night?" or a subordinate comments, "You look stressed today," even though you're in a perfectly good mood!


The answer lies in the upper part of your face... The eyes are indeed the windows to the soul, but the "brows and eyelids" are the "window frames." If the frame is rotten and deteriorated, no matter how bright the inside is, outsiders will only see the deterioration.


Over my 14 years in the medical field, from the emergency rooms of public hospitals to top-tier plastic surgery operating theaters, I have met thousands of patients distressed by this issue, and most have been "fixing the wrong spot" all along!


Some bought an expensive cream jar for thousands... (their eyelids didn't lift). Some got thread lifts until their face was wavy... (and in 3 months, it sagged again). Some had double eyelid surgery, but their face looked angrier than before...

To help you understand why I choose certain treatments, we need a little bit of science (I promise it won't be boring).


Our face consists of 3 main layers that change over time:


1. The Bone Layer (The Foundation): A house with a sinking foundation

You might not know that as we age past 30-40, our "skull bones" undergo atrophy (Bone Resorption), especially around the Orbital Rim (eyebrow bone) and the temples.

The Result: When the bone foundation shrinks, the tissues, brows, and eyelids that were once attached are like a house with sinking soil; they slide down and accumulate below. This causes deep-set eyes and drooping eyelids.


2. The Muscle and Fascia Layer (The Structure): Stretched-out elastic bands

Our forehead has a large muscle called the Frontalis, which pulls the brows up. But as we age, the Retaining Ligaments that anchor the brow to the bone slacken like old rubber bands.

The Result: The brows can't fight gravity and drop down (Brow Ptosis), pushing the eyelid skin to cover the eyes.


3. The Skin Layer (The Surface): The sagging curtain

The eyelid skin is the "thinnest" skin on the human body. It blinks tens of thousands of times a day. Over time, collagen is lost, elastin degrades, and it stretches and sags (Dermatochalasis).

The Result: Excess skin piles down and covers the eyelid crease. For some, it's severe enough to block the pupil, leading to impaired vision, requiring them to look up or lift their brows constantly, causing headaches.

Before walking into a clinic, try this Self-Check in the mirror:


Type A: Eyelid Hooding

  • The brow is still in its original position (above the orbital rim).

  • But there is excess, sagging eyelid skin covering the crease.

  • The Solution: Excess skin removal / Double Eyelid Surgery (Blepharoplasty) rightarrow Done! Beautiful!


Type B: Brow Ptosis

  • Try lifting your brow to a preferred position with your hand; if the sagging eyelid disappears and your eyes look softer.

  • The distance between your "eye" and "brow" is very narrow (less than 2 cm).

  • You have many wrinkles on your forehead (from constantly raising your eyebrows to help).

  • The Solution: Absolutely Do NOT get double eyelid surgery! Because if you remove eyelid skin, the brow will drop even further, making your face look very stern. The only solution is a "Brow Lift."


Type C: Both (Combination)

  • This is the most common case in Thais aged 45+; both the brow is low, and there is excess eyelid skin.

  • The Solution: Requires a Hybrid Technique, which is Dr. Teng's specialty.

CHAPTER 3: Aurora’s Signature Techniques – Unveiling Dr. Teng's 3 Master Moves

  • Principle: I make an incision "just below the bottom edge of the eyebrow hair" to remove excess eyelid skin and fat, then lift the sagging skin to tighten it.

  • Why Subbrow Lift?

    • Preserves Original Look: If you already like your existing double eyelid crease (or don't want a new eye crease to change your face), this method "restores" the original crease definition.

    • Excellent for Outer Eye Sagging: For those with droopy outer corners or a sad appearance, this method lifts the outer eye beautifully.

    • Invisible Scar: With my suturing technique, the scar hides seamlessly along the brow line. If you wear makeup or have eyebrow tattooing, the scar is virtually invisible. 100% of patients say, "Friends couldn't tell I had surgery, only that I look better."

    • Dr. Teng's Special Technique: I perform Internal Fixation, which involves suturing the underlying muscle layer to the periosteum (bone membrane) to act as an "anchor" and lock the brow in place, preventing future sagging. (Some doctors only cut the skin, but I stitch the inside for durability.)

  • Principle: Removing skin "above the eyebrow" to lift the entire brow unit higher.

  • Why Direct Brow Lift?

    • The most direct solution for those whose brow is significantly lower than the standard.

    • Excellent at removing forehead wrinkles and crow's feet.

    • The face will instantly look "brighter" and "more open."

    • Fear of Scars? I understand the fear of facial scarring. I use the Trichophytic Incision technique (a 45-degree angled cut) to preserve the hair roots at the incision edge. When the wound heals, new eyebrow hairs will grow through the scar, helping to camouflage the line perfectly. Coupled with Tension-Free Suturing, the scar heals beautifully and fades very quickly.

  • Principle: A small incision is made and hidden in the "hairline" at the temple. Instruments are then inserted to lift the SMAS layer (the superficial fascia beneath the skin) in a diagonal direction to tighten.

  • Why Temporal Lift?

    • Scarless on the Face: The scar is concealed in the hair. You can tie your hair up and still not see it.

    • Foxy Look: Creates sharp, defined eyes (Cat Eye) which is a global trend.

To those who fear surgery, I will walk you through a simulation to ease your worries.


Step 1: Consultation

I will have you sit in front of the mirror and use tools to simulate the brow lift. We will discuss the shape, expectations, and possibilities. I will be completely honest. If something will look great, I'll say so; if it won't work, I will advise against it.


Step 2: Design & Marking

This is the most crucial step. I will spend time meticulously drawing lines on your face, measuring down to the millimeter to ensure the two sides are as close as possible. (Naturally, human faces are not 100% symmetrical, but I will make them as balanced as possible).


Step 3: Anesthesia (The Numbing That Doesn't Hurt as Much as You Think)

I know everyone fears pain... so I use a special technique.

  • Topical numbing cream is applied first.

  • Extreme cold compression is applied until the skin is numb.

  • I use the smallest needle (as thin as a hair) to slowly administer the anesthetic. 99% of patients tell me, "It only stung a tiny bit with the first needle." After that, you just relax and listen to music. No general anesthesia is required; it's safer and recovery is faster.


Step 4: The Art of Surgery

I spend about 45-90 minutes calmly performing the surgery, dissecting the tissue layers, removing the excess, and stitching the muscle layers. We can talk and joke throughout the procedure. I will always let you know, "I'm about to cauterize," or "I'm about to suture."


Step 5: The Reveal (The Moment of Truth)

Once suturing is complete, I will immediately hand you a mirror... That moment is when I feel the happiest. Seeing the patient smile because the heavy feeling in their eyes is instantly gone and their eyes are wide and comfortable.

CHAPTER 6: Recovery Guide – Post-Surgery Care Manual (For Fast, Beautiful Scars)

Surgery is 50% my job; the other 50% is the patient's job in wound care.

  • The First 3 Days (The Golden Time): COLD COMPRESS! Compress as often as possible. Cold helps stop bleeding and significantly reduces swelling.

  • Sleeping: Elevate your head with 2-3 pillows during the first 2 nights to prevent blood from pooling in the face.

  • Cleaning: Do not let the wound get wet for 24 hours. Afterward, gently clean with saline solution and apply the ointment I provide (the wound should not be too dry or too wet).

  • Diet: Avoid fermented foods, Pla Ra (fermented fish), and alcohol for 2 weeks (they can cause itching and swelling). Eat pumpkin, Bai Bua Bok (Centella Asiatica) juice, and boiled eggs to your heart's content; they help wound healing.

  • Stitch Removal: Come back after 5-7 days for stitch removal. It's painless and quick.

CHAPTER 7: FAQ – Answering Your Burning Questions (That People Don't Dare to Ask)

Q: Doctor, I'm Southern Thai and already have sharp eyes. Will this make my face look angry?

A: This is why Subbrow Lift is highly suitable for Southern Thais. We don't interfere with the original eye crease; we simply remove the skin that is blocking the eye, allowing your naturally sharp eyes to shine again without creating a thick, stern-looking crease.

Q: I get keloids easily. Can I still have the procedure?

A: The good news is that the skin around the eyelids and brows is the "least likely" area on the body to form keloids. The chances are extremely low. I have almost never encountered it. But if you are concerned, I have preventative topical medication for you.

Q: How long does the result last?

A: Since the surgery physically removes excess tissue, the results are long-lasting, typically 5-10 years, or until natural aging causes new sagging (which is a very long time). It is definitely more cost-effective than Botox or thread lifts that need annual repetition.

Q: I'm on Koh Samui. Should I cross over to Surat Thani or have it done on Samui?
