"Wrinkled, Sagging Face?" How to Regain Youth... "Facelift" is the Answer: More Than Just a Solution, It's the Art of Personal Beauty Creation by Dr. Teng from Aurora Clinic.

Hello everyone, I'm Dr. Apiruk Wongsaowasup, or as many know me, "Dr. Teng," the owner of Aurora Clinic, a plastic surgery and aesthetic clinic in Surat Thani and Koh Samui. Throughout my career in plastic surgery, I've met countless individuals seeking consultations due to concerns about changes in their facial appearance, especially the issues of a "wrinkled, sagging face" and a "crepey neck," which are clear signs of aging and significantly impact daily confidence.

Today, I want to invite you all to uncover the secrets and understand the journey to lasting youth through the science of Facelift surgery. From my perspective, this isn't just a surgical procedure to correct sagging; it's the "creation of personalized beauty," the restoration of balance, and the revitalization of your hidden confidence. At Aurora Clinic, I prioritize natural-looking results, utmost safety, and the highest satisfaction for every patient.

Lifting the Veil of Concern: When Time Leaves Its Mark on the Face and Neck

Lifting the Veil of Concern: When Time Leaves Its Mark on the Face and Neck
Lifting the Veil of Concern: When Time Leaves Its Mark on the Face and Neck

In the consultation room at Aurora Clinic, I've met many people who come to me with similar feelings. Some walk in with tired, dull faces; some slowly caress the sagging lines of their jawline and neck; and many begin the conversation with words that reflect deep concern, such as, "Doctor, why is my face so saggy?" "My laugh lines are so deep, I look much older than my actual age." "I feel like my face is sliding down and pooling around my neck," or "My neck is so wrinkled I don't dare wear open-neck shirts."

These words aren't just complaints about external skin problems; they're echoes of an inner feeling eroded by obvious signs of aging. The face and neck, always exposed and visible, become the clearest "indicators" of increasing age. When external appearance doesn't align with an inner feeling that remains vibrant and energetic, it inevitably leads to self-consciousness and a diminished quality of life.

I understand where these concerns come from and how they arise, because the aging process is something everyone faces, but the severity and timing of its appearance vary from person to person, with many contributing factors. Let's delve into the causes and characteristics of "wrinkled, sagging face" and "crepey neck" problems.

1. Sagging Skin on the Face and Neck: Battling Gravity and Subcutaneous Changes

Sagging Skin on the Face and Neck: Battling Gravity and Subcutaneous Changes
Sagging Skin on the Face and Neck: Battling Gravity and Subcutaneous Changes

This is the core of the problem of "wrinkled, sagging face" and "crepey neck," which arises from complex processes beneath our skin.

The Decrease in Collagen and Elastin

As we enter our mid-20s, our bodies begin to produce less of two crucial proteins that form the main structural components of the skin: "collagen," which provides strength and plumpness, and "elastin," which gives elasticity and helps the skin bounce back. This reduction causes the skin to lose its firmness, tautness, and elasticity, leading to the appearance of fine lines and gradual sagging.

Gravity

This is a formidable enemy that works continuously and relentlessly. Gravity constantly pulls our skin and underlying tissues downwards. Over decades, the cumulative effect of gravity causes the fatty tissue and muscles on the face, once in higher positions, to gradually move lower, resulting in noticeable sagging.

Bone Resorption and Facial Fat Changes

This is a cause many overlook but is extremely important. As we age, our facial bone structure changes and resorbs, especially around the temples, eyes, cheekbones, and jawline. This resorption reduces the space for soft tissues, making the face appear hollow and losing its fullness. Additionally, our facial fat also changes; some of it atrophies (Fat Atrophy), making the face look gaunt, while some moves downwards (Fat Descent) to accumulate in undesirable areas, such as the lower cheeks and jowls. When the underlying support structure weakens and fat shifts, the overlying skin loses its support and sags accordingly.

Repetitive Facial Expressions and Facial Muscles

Our repeated facial expressions, such as smiling, laughing, frowning, or raising eyebrows, over many years lead to "dynamic wrinkles." As the skin loses collagen and elasticity, these wrinkles become deeply etched and turn into "static wrinkles" that are clearly visible even when we are not making expressions.

External Factors and Lifestyle

In addition to natural processes, external factors and lifestyle habits significantly accelerate sagging and wrinkles. These include prolonged exposure to ultraviolet (UV) radiation from intense sunlight, which directly damages collagen and elastin; smoking, which reduces blood flow to the skin; air pollution; dehydration; lack of sleep; and an unhealthy diet. All these factors stimulate premature skin aging.

2. Deep Grooves and Pronounced Wrinkles: Lines That Tell the Story of Time

Deep Grooves and Pronounced Wrinkles: Lines That Tell the Story of Time
Deep Grooves and Pronounced Wrinkles: Lines That Tell the Story of Time

As sagging occurs, wrinkles and deep grooves become more evident in various areas of the face:

  • Nasolabial Folds: Also known as smile lines, these are deep grooves that run from the sides of the nose down to the corners of the mouth. They result from the sagging of the skin and fat in the mid-cheek area, making the face look tired, older, and sometimes even sad.

  • Marionette Lines: These are deep grooves that extend from the corners of the mouth down to the chin and jawline. They often make the corners of the mouth appear downturned, giving the face a grumpy, sad, or perpetually angry look, even if one doesn't feel that way. These wrinkles can convey incorrect emotions to others.

  • Forehead Wrinkles and Frown Lines: (as extensively discussed in a previous article) Horizontal wrinkles on the forehead and vertical lines between the eyebrows are another clear sign of increasing age, resulting from both repetitive facial expressions and skin laxity.

3. Jowls and Turkey Neck: When Youthfulness Is Obscured

Jowls and Turkey Neck: When Youthfulness Is Obscured

These two areas are another clear age indicator often overlooked in self-care, yet they significantly impact confidence:

  • Jowls: Occur when the skin and fat in the lower cheek and jawline area sag, leading to a loss of the sharp definition of the jawline that was once crisp in youth. This makes the lower face appear heavy, droopy, and contributes to an older appearance.

  • Crepey Neck (Turkey Neck / Platysmal Bands): The skin on the neck is much thinner and more prone to dryness than facial skin, making it susceptible to wrinkles and sagging. When the Platysma muscle (a superficial muscle in the neck area) becomes lax and separates, it appears as vertical bands on the neck called Platysmal Bands. Additionally, the accumulation of fat under the chin and on the neck combined with sagging skin can lead to a "turkey neck," a problem that significantly adds to an aged appearance and often makes many people hesitant to wear open-neck shirts or clothing that reveals the neck area.

Beyond Appearance: When Confidence Is Shaken from Within

Beyond Appearance: When Confidence Is Shaken from Within
Beyond Appearance: When Confidence Is Shaken from Within

These issues aren't just about external physical changes; they also profoundly affect the patient's psychological state and inner feelings, ultimately impacting their overall quality of life inevitably:

  • Severe Erosion of Self-Confidence: When external appearance changes in a way that causes dissatisfaction, and the face in the mirror doesn't reflect an inner feeling that remains vibrant, patients often feel self-conscious. They may avoid eye contact, shy away from photos, or avoid social interactions because they feel their face is unattractive or looks older than their age.

  • Impact on Work and Career: In many professions that require strong personal presence, communication, or making a good first impression, such as management, service, sales, or even executive roles, a tired, sagging face or a crepey neck can unintentionally affect credibility, enthusiasm, and the impression others have of us.

  • Accumulated Stress and Anxiety: Frequent comments about facial changes, the constant need to try and conceal issues with makeup, or a general dissatisfaction with one's appearance can lead to stress, anxiety, and self-dissatisfaction, impacting overall mental health and potentially leading to depression.

  • Reduced Enjoyment of Life: When constantly feeling self-conscious or worried, one might miss out on good opportunities in life, hesitate to engage in favorite activities, or be unable to live life as fully and happily as they should.

I, Dr. Apiruk Wongsaowasup, or Dr. Teng, deeply understand these distresses and concerns, and I recognize that resolving "wrinkled, sagging face" and "crepey neck" issues isn't merely about "beauty"; it's about restoring "happiness," "confidence," and "quality of life" to every patient.

At Aurora Clinic in Surat Thani and Koh Samui, my entire team and I are committed to being a part of this journey. I'm ready to listen, understand, and offer the best, safest, and most natural treatment options, so you can return to being your best self, with a youthful, radiant face, and brimming with confidence once again.

"Every Face is a Work of Art": Dr. Teng's Philosophy of Beauty Creation at Aurora Clinic

"Every Face is a Work of Art": Dr. Teng's Philosophy of Beauty Creation at Aurora Clinic
"Every Face is a Work of Art": Dr. Teng's Philosophy of Beauty Creation at Aurora Clinic

I've dedicated myself to the art of plastic surgery for many years. Along this path, I haven't viewed my work merely as "surgery" to fix skin laxity or "repairing" imperfections caused by time. Instead, I see it as a delicate, intricate process that requires both the precise science of medicine combined with the art of creating beauty.

This is the origin of the philosophy I've always upheld, and it forms the fundamental basis of my and my team's work at Aurora Clinic: "Every face is a work of art." For me, true art is beauty that harmonizes with nature, beauty that reflects each individual's unique identity, and beauty that generates lasting happiness and confidence from within.

Imagine a great artist creating a masterpiece sculpture. They don't just haphazardly carve stone; they meticulously plan, deeply understand the characteristics of the stone, comprehend the anatomy of what they wish to sculpt, and utilize their expertise to remove excess and fill in what's lacking to achieve a form that conveys emotion and vitality. Similarly, in my work with Facelift to restore youthfulness to each patient's face, I apply these same principles and diligently hone my skills, continuously advancing my knowledge to ensure that every outcome is the epitome of precision and an understanding of aesthetic beauty.

The essence of my "Every face is a work of art" philosophy in creating beauty through Facelift encompasses several important dimensions:

1. Deep Understanding of the "Canvas" and "Unique Personal Identity" (The Canvas and Its Unique Features: A Holistic View)

Deep Understanding of the "Canvas" and "Unique Personal Identity" (The Canvas and Its Unique Features: A Holistic View)
Deep Understanding of the "Canvas" and "Unique Personal Identity" (The Canvas and Its Unique Features: A Holistic View)

Before embarking on any artistic creation, the first thing an artist must do is deeply understand their "canvas." In plastic surgery, my "canvas" is each patient's face, which is entirely unique. No two faces are 100% identical; each has its own bone structure, the shape of their eyes, nose, chin, facial proportions, skin elasticity, and other specific characteristics that make each person unique. Most importantly, there's the "story" that time has left on their face and neck.

I spend a significant amount of time discussing, taking a detailed history, and most importantly, "listening" deeply to each patient's genuine needs, concerns, and expectations. I will inquire about their feelings regarding the problems they're facing, such as concerns about a wrinkled, sagging face that makes them look older than their age, a crepey neck that erodes their confidence, or the feeling that their face is "sliding down" and losing its defined contour. A thorough understanding of these "desires" is the starting point for creating truly satisfying and natural-looking results.

I meticulously analyze each patient's facial structure from every dimension, including visual observation, palpation, using photos from various angles, and employing modern tools. This allows me to identify the true problems, the underlying causes, and most importantly, the "potential" for rejuvenation within that specific face. I don't just see the "problem" to be fixed, but rather the "opportunity" to create beauty in their own way, while preserving their true self and original identity. Crucially, it's essential to understand that a good Facelift must consider the upper, middle, and lower face, as well as the neck, for overall harmony.

2. Meticulous and Precise "Blueprint" Planning (The Blueprint: Meticulous Planning for Natural Results)

After deeply understanding the canvas and its unique features, the next equally crucial step is "planning." An artist begins by sketching to define proportions, composition, light and shadow, and the direction of the work. In plastic surgery, especially for a Facelift, this is the stage of the most meticulous and precise treatment planning. I combine the information obtained from analysis and discussions with the patient with my accumulated knowledge and experience to design a personalized treatment "blueprint." This blueprint may include:

  • Selecting the Most Suitable Facelift Technique: Whether it's a Mini Facelift, Standard Facelift (SMAS Facelift), or a specific Neck Lift, I will choose the technique that yields the best, safest, and most appropriate results for each patient's condition and needs. I don't just use the most modern techniques; I use the technique that is "right" for that particular patient, which means precisely managing the skin layer, fat layer, and muscle/fascial layer (SMAS/Platysma) for lasting results.

  • Defining Clear and Achievable Goals: I will discuss realistic and achievable outcomes with the patient, based on their own structural characteristics. There are no over-promises; instead, I provide complete and transparent information to ensure the patient has a correct understanding and doesn't have unrealistic expectations. The goal is natural youthfulness, not becoming a completely different person.

  • Designing Surgical Incisions and Scar Concealment: With an understanding of art, I meticulously design the surgical incisions, often hiding them along the hairline, in front of the ear, or behind the ear. Once healed, the scars will be minimally visible, almost imperceptible, which is a crucial aspect of a natural-looking Facelift.

  • Considering Ancillary Procedures for Comprehensive Results: Planning may also include liposuction to remove jowls and neck fat, autologous fat grafting to replenish lost volume, or using botox and fillers to refine wrinkles and add dimension. This is a holistic approach to achieve the most perfect results.

  • Safety as the Foremost Priority: In every step of planning and execution, patient safety is paramount. I thoroughly assess the patient's overall health, carefully review their medical history, and provide necessary advice to ensure the treatment proceeds smoothly and safely.

This meticulous planning is essential for achieving beautiful and safe results, much like preparing a strong foundation for a masterpiece.

3. Delicacy and the Art of Creation (The Brushstrokes and Sculpting: Precision and Artistry)

Delicacy and the Art of Creation (The Brushstrokes and Sculpting: Precision and Artistry) | Facelift by Dr. Apiruk, Plastic Surgeon

Once the blueprint is ready, the artist begins to use their tools to create the masterpiece. In the Facelift procedure, I apply the highest level of skill, knowledge, experience, and precision to ensure every step of the surgery is as accurate and delicate as possible.

  • Precise and Delicate Incisions: Making the smallest necessary incisions, gently handling tissues, and meticulous suturing are all crucial. I use appropriate instruments for each step to minimize damage to surrounding tissues, reduce swelling and bruising, and accelerate recovery.

  • Artful Management of Deep Layers: The key to a natural and long-lasting Facelift is the management of the SMAS layer and the Platysma muscle. I meticulously lift and reposition these deep tissues and muscles to their youthful position, rather than just simply tightening the skin. The latter is the cause of faces that appear stiff, unnatural, or "fake."

  • Restoring the Jawline and Neck: I place special emphasis on creating a sharp jawline and a smooth neck, free from crepey neck or jowls, which are clear indicators of youthfulness. Managing excess fat and properly rearranging the neck muscles is an art that requires deep experience and understanding.

  • Blending Science and Art: Facelift surgery isn't just about performing a textbook procedure or following steps. It requires the "eye" of an artist to perceive the balance, harmony, and hidden beauty. I evaluate and refine every millimeter to ensure that the results not only correct the problem but also enhance the patient's natural facial dimensions and beauty, making them even more distinct and harmonious.

4. "The Masterpiece": Natural Harmony and Lasting Youth

"The Masterpiece": Natural Harmony and Lasting Youth | Facelift by Dr. Apiruk, Plastic Surgeon

The ultimate goal of good art is to convey emotion and vitality. For me, the best "work of art" on a patient's face is beauty that looks natural—not stiff, artificial, or overly "done." What I aspire to is for patients to be able to live confidently again, with those around them noticing that "you look better," "you look more vibrant," "you look younger," but unable to pinpoint exactly "what you had done." This is a truly successful Facelift.

  • Emphasizing Harmony and Balance: I focus on improving and enhancing existing beauty, aiming for the overall face to appear balanced and harmonious in all proportions, from the forehead to the neck. It's not about transforming into a new person, but about bringing out the unique beauty potential of each patient in their own distinct way.

  • Lasting and Durable Results: A good Facelift should yield results that last for many years because we're not just pulling skin; we're addressing deep structural layers. I provide detailed advice on post-treatment self-care to ensure the results endure as long as possible.

  • Patient Happiness and Confidence: Ultimately, the most important and invaluable result for me is the patient's joyful smile, their regained confidence, and their improved quality of life. Seeing patients return to their daily lives with confidence, daring to smile, daring to make eye contact, and daring to be fully themselves – that's what confirms the true success of my "work of art" and is why I love plastic surgery.

At Aurora Clinic in Surat Thani and Koh Samui, my team and I strictly adhere to this "Every face is a work of art" philosophy. We believe that everyone deserves the best beauty, utmost safety, and the meticulous care of an artist creating a masterpiece, so that your journey to a youthful face once more, a Facelift to address wrinkled, sagging face and crepey neck issues, is smooth and filled with happiness.

"Facelift" is the Answer: The Science and Art of Sustainable Rejuvenation

After discussing the issues and concerns about facial changes, particularly the obvious signs of aging like a "wrinkled, sagging face" and a "crepey neck," which significantly impact daily confidence, it's time to delve into the "answer" that provides the clearest, most comprehensive, and most lasting anti-aging results: the Facelift.

A Facelift isn't merely a "surgical procedure" to correct sagging; it's a complex process that combines both the "science" of precise medicine and the "art" of beauty creation in perfect harmony. This is the creation of lasting youthfulness, the restoration of balance, and the reawakening of the confident energy hidden within you, bringing it back to life. This is my approach at Aurora Clinic, focusing on natural results, maximum safety, and the highest patient satisfaction.

The Science of "Facelift": Anatomical Precision and Surgical Techniques

As a plastic surgeon, I adhere strictly to scientific and anatomical principles to ensure the safety and effectiveness of every Facelift procedure:

  • In-depth Anatomical Knowledge: A good Facelift begins with a thorough understanding of the human facial structure, from the skin layer, fat layer, muscle and fascia layer (SMAS - Superficial Musculoaponeurotic System and Platysma muscle in the neck area), down to the facial bone structure, nerves, and blood vessels. I study and understand the relationships of each tissue layer in detail to perform surgery precisely, avoid injury to important nerves and blood vessels, and properly manage each tissue layer for safe and natural results.

  • Understanding Aging Processes: I deeply understand the true causes of "wrinkled, sagging face" and "crepey neck." It's not just skin laxity, but changes occurring at every level: the reduction of collagen and elastin, facial bone resorption, fat atrophy and migration, and the sagging of the SMAS layer and Platysma muscle. This understanding allows me to plan surgical procedures that address the problem precisely at every depth, not just superficially tightening the skin.

  • Proven Surgical Techniques: I use Facelift techniques that are internationally recognized and scientifically supported, whether it's techniques focusing on SMAS layer lifting (SMAS Facelift), which is the core of a Facelift that provides lasting and natural results, or other specific techniques suitable for individual problems. Every step is performed under strict control, including sterile conditions, appropriate local/general anesthesia, and patient health monitoring throughout the surgery.

  • Precise Fat and Muscle Management: For prominent jowls and crepey neck problems, excess fat accumulation and Platysma muscle laxity in the neck area are often involved. I will perform liposuction to remove excess fat and repair or tighten the Platysma muscle (Platysmaplasty) to create a slender, firm jawline and neck, which is a crucial part of making the face look youthful and defined.

The Art of "Facelift": Creating Personalized Beauty

Beyond scientific knowledge, a good Facelift requires the "eye" of an artist and the "skill" to create beauty that harmonizes with each individual's natural features:

  • Aesthetic Vision: I envision the overall face and neck in its most youthful and beautiful form for each patient, not by merely "pulling tight" haphazardly, but by "lifting and repositioning" sagging tissues to return to their youthful, natural position, while preserving the patient's unique identity and style. This prevents them from looking like a different person or appearing stiff like a mannequin.

  • Artistic Incision Placement: This is where the delicacy of art plays a crucial role. I meticulously and cleverly place the surgical incisions to hide the scars in the least visible positions, such as along the hairline, in front of the ear, or behind the ear. Once healed, the scars will blend with the skin, almost imperceptibly, which is extremely important for the patient's long-term confidence.

  • Meticulous Contouring and Sculpting: Every step of the surgery, from lifting the SMAS layer, removing excess fat, repositioning tissues, and closing incisions, must be performed with the utmost precision. I use delicate hands and keen eyes to fine-tune every millimeter to achieve a sharp jawline, lifted cheeks, shallower laugh lines, and a smooth neck, free from jowls or crepey neck, creating a harmonious connection between the face and neck.

  • Achieving Balance and Harmony: The ultimate goal is a natural-looking result. A Facelifted face should appear brighter and younger, but still retain the patient's identity, without looking so changed that people don't recognize them. I will prioritize the balance of the entire face, ensuring that the corrected areas blend harmoniously with other parts and appear as natural as possible.

1. Pre-Facelift Assessment and Planning: The Heart of Personalized Beauty Creation

Embarking on the journey to youthfulness with a Facelift at Aurora Clinic begins with the most crucial step: "assessment and planning." I consider this to be the very heart of creating personalized beauty.

In-depth Consultation and Active Listening

I dedicate ample time to speak with each patient, gaining a deep understanding of their concerns about a "wrinkled, sagging face" and a "crepey neck," as well as their true expectations from the procedure. I'll ask questions to understand their lifestyle, profession, and the feelings they wish to regain, because true beauty lies in happiness and regained confidence.

Holistic Facial Analysis

I perform a detailed and comprehensive analysis of each patient's face, considering the upper face (forehead, eyebrows), midface (around the eyes, cheekbones, laugh lines), lower face (cheeks, jowls, jawline), and neck, including how each part connects. I'll evaluate skin condition, fat layers, sagging muscles, and bone structures that may have changed. A 3D assessment (if necessary) helps me clearly visualize the entire structure.

Setting Realistic and Achievable Goals

I'll explain the limitations and realistic expected outcomes based on the patient's own structural characteristics and problem areas. I won't make over-promises, but I will provide complete and transparent information to ensure patients have a correct understanding and don't have unrealistic expectations. The goal is: "You'll look naturally younger and more vibrant," not "You'll be a completely different person."

Customized Surgical Blueprint Design

Using all the gathered information, I'll create a "blueprint" or surgical plan specifically tailored for each patient. This plan will specify the chosen technique, the hidden placement of surgical incisions, the amount of fat that might need to be removed, or areas that might require additional fat injection, along with the plan for managing the SMAS layer and Platysma muscles to achieve the most harmonious and natural results.

Considering Ancillary Procedures for Comprehensive Results

Planning also includes considering other complementary procedures, such as liposuction for the jowls and neck, or autologous fat grafting to replenish lost volume, to achieve the most comprehensive and complete rejuvenation results.

2. Diverse Facelift Techniques by Dr. Teng: Choosing the Best for You

Diverse Facelift Techniques by Dr. Teng: Choosing the Best for You | Facelift by Dr.Apiruk, Plastic Surgeon

At Aurora Clinic, I specialize in a variety of Facelift techniques, allowing me to choose the most suitable procedure for each patient's specific concerns and needs. My focus is always on clear, safe, and natural results:

Mini Facelift (Mini-Lift)

  • Ideal for: Individuals with mild to moderate sagging, especially in the lower cheek (jowls) and jawline that is beginning to lose definition. It's also suitable for those who don't want a major surgery or who seek targeted correction with faster recovery.

  • My Technique: I make smaller incisions than a full Facelift, usually hidden in front of the ear or along the hairline in front of the ear. The focus is then on lifting and tightening the SMAS (Superficial Musculoaponeurotic System) layer, which is the muscle and fascia layer directly beneath the skin. Tightening this layer effectively addresses sagging cheeks and jowls along the jawline, creating a sharper jawline without excessive skin pulling.

  • Benefits: Smaller incisions lead to less pain, faster recovery, and significantly less swelling and bruising. It provides natural and effective anti-aging results, ideal for a "refresh" of the face in the early stages of sagging.

Standard Facelift (SMAS Facelift)

  • Ideal for: Individuals with significant and comprehensive sagging of the face and neck, including the cheeks, deep nasolabial folds, jowls, and a crepey neck, who desire the most noticeable, comprehensive, and long-lasting anti-aging results.

  • My Technique: This is a procedure I am highly specialized in and is central to a Facelift that provides lasting results. I make incisions hidden along the hairline, in front of the ear, and extending behind the ear to allow extensive access to the skin layer and, importantly, the SMAS layer. I then meticulously lift and reposition the SMAS layer and the Platysma muscle in the neck to naturally restore sagging skin and deep tissues to their original, youthful position. Simultaneously, I remove excess fat that contributes to jowls or a crepey neck and precisely trim excess skin to ensure smooth, taut skin without sagging.

  • Benefits: Provides the most visible and comprehensive lifting and tightening results across the midface, lower face, and neck. It effectively reduces nasolabial folds, marionette lines, and corrects jowls and a crepey neck with maximum effectiveness, with results lasting for many years. It genuinely creates a youthful and natural appearance.

Neck Lift: Specifically Addressing "Crepey Neck" and Jowls

  • Ideal for: Individuals primarily concerned with a crepey neck and jowls, where the upper face does not show significant sagging, or as a combined procedure with a Mini Facelift or Standard Facelift for more complete neck rejuvenation, especially for those with prominent Platysmal Bands (vertical lines on the neck).

  • My Technique: I make a small incision under the chin, and there may be a hidden incision behind the ear, to access the lax Platysma muscle. I then tighten this muscle (Platysmaplasty), which is crucial for correcting a crepey neck. Additionally, excess fat in the neck area is removed (Neck Liposuction), and excess neck skin is precisely trimmed.

  • Benefits: Effectively corrects a crepey neck, jowls, and vertical neck lines, resulting in a visibly smoother neck, a sharper jawline, and a more youthful appearance that harmonizes with the face.

In every step, I, Dr. Apiruk Wongsaowasup, closely supervise to ensure that the results are not only aesthetically pleasing but also natural-looking, and most importantly, safe for every patient. This is the science and art of Facelift that I am proud to offer to all at Aurora Clinic in Surat Thani and Koh Samui.

"Safety is the Heart": Dr. Teng's Patient Care Philosophy at Aurora Clinic

Lift to Turn Back Time! Consult Before Facelift Surgery, Get Ready for Amazing Results!

Throughout my career as a plastic surgeon, I have consistently prioritized one thing above all: "patient safety." I firmly believe that no matter how excellent the aesthetic results may be, if they lack a strong foundation of safety, they are worthless and may lead to distress later on.

Especially for complex and significant procedures like a Facelift or the correction of a "wrinkled, sagging face" and a "crepey neck," meticulous planning and execution under the highest safety standards are absolutely essential. At Aurora Clinic, we don't view safety merely as a regulation to be followed, but as a "philosophy" deeply embedded in every process, every decision, and every detail of patient care, from the moment they first step in for a consultation until their post-treatment follow-up.

I'll explain the principles and safety measures that my team and I uphold and strictly adhere to, so that all patients can be confident they will receive the best and safest care.

1. Thorough Pre-operative Health Assessment

Before deciding on any procedure, especially a Facelift, which requires particular caution, I conduct a detailed and comprehensive health assessment of the patient in every dimension. This is the first and most crucial step in risk prevention:

In-depth Medical History Taking

I thoroughly inquire about the patient's health history, including pre-existing conditions (such as diabetes, high blood pressure, cardiovascular disease, easy bleeding/clotting disorders), previous surgeries, history of drug or food allergies, current regular medications, including herbal remedies and supplements, smoking, and alcohol consumption. All of these factors affect surgical safety and recovery.

Necessary Physical Examination and Laboratory Tests

To ensure the patient is medically fit for surgery, I may perform additional physical examinations, such as blood pressure measurement, cardiac evaluation, lung examination, and detailed blood tests (e.g., CBC, PT, PTT, liver and kidney function tests, blood sugar levels), as well as an electrocardiogram (EKG) and chest X-ray. If any abnormalities or pre-existing conditions are found, I may consult with an internist or other specialists and request a certificate of fitness to ensure the patient is suitable for surgery and anesthesia.

Risk Assessment and Transparent Explanation

I explain potential risks in detail and in easy-to-understand language. For a Facelift, risks may include bleeding, infection, nerve injury, visible scarring, or complications from anesthesia. I'll clarify preventive measures and management plans should these issues arise, so that patients understand all aspects and can make an informed decision with complete peace of mind.

2. Expert Surgeons and Experienced Team

The knowledge, expertise, and experience of the medical team and staff are the most crucial factors in the safety of Facelift surgery:

Expert Plastic Surgeon

Every step of the procedure is performed by myself (Dr. Apiruk Wongsaowasup, Medical Council No. 35777), a board-certified plastic surgeon with extensive experience in facial surgery, particularly Facelifts. I've accumulated experience from a wide range of cases, which allows for precise problem analysis, meticulous planning, and delicate surgical execution, minimizing the risk of complications.

Expert Anesthesiologist Team

For Facelift procedures, which often require general anesthesia or local anesthesia combined with sedation, we have a team of certified Anesthesiologists who closely monitor and control the administration of anesthesia/sedation, as well as the patient's vital signs throughout the surgery. This ensures the patient is comfortable, pain-free, and safe throughout the process.

Well-Trained Nurses and Surgical Assistants

Every nurse and surgical assistant at Aurora Clinic undergoes intensive training, possesses a deep understanding of each procedure, and has experience assisting surgeons during operations. This ensures that every step proceeds smoothly and with maximum efficiency.

3. Standardized and Sterile Operating Rooms

A clean and sterile environment is crucial for preventing infection, which is one of the most serious complications of surgery:

Hospital-Level Standards

The operating rooms at Aurora Clinic in Surat Thani and Koh Samui are designed and maintained to high standards of cleanliness and sterility, comparable to hospital operating rooms. They feature controlled temperature, humidity, and air circulation with HEPA filter systems to minimize airborne pathogens.

Strict Instrument Sterilization

All medical instruments and equipment undergo sterilization processes with the highest standards and are regularly inspected to ensure they are truly free of pathogens.

Maintaining Sterility During Surgery

All personnel in the operating room wear sterile surgical gowns and other protective equipment strictly to maintain sterility throughout the surgical procedure.

4. Modern Equipment and Technology

The use of modern technology and equipment enhances surgical precision, efficiency, and safety:

Delicate Surgical Instruments

We select modern and highly delicate surgical instruments and equipment specifically designed for facial surgery. This allows for precise work on delicate tissues, minimizing damage to surrounding tissues.

Vital Signs Monitoring Equipment

In the operating room, modern vital signs monitoring equipment (e.g., EKG, blood pressure monitor, pulse oximeter) continuously monitors the patient's condition in real-time. This allows for immediate detection of abnormalities and prompt intervention.

Standardized Products and Medications

We select medical products, medications, and surgical materials (e.g., sutures) that are certified by the Food and Drug Administration (FDA) in both Thailand and internationally, to ensure quality, safety, and effectiveness.

5. Close and Continuous Post-operative Care

Surgery is only half the battle; post-operative care is the equally important other half and contributes significantly to beautiful results and safe recovery:

Care in the Recovery Room

After surgery, patients receive close monitoring in the recovery room. The nursing team continuously monitors vital signs, pain, and swelling to ensure the patient recovers safely from anesthesia/sedation and feels as comfortable as possible.

Detailed Self-Care Instructions

My team and I provide detailed and clear post-operative self-care instructions, both in written form and verbally. This includes how to clean wounds, proper cold/warm compresses, taking prescribed medications, limiting activities, resting, sleeping in appropriate positions, and other do's and don'ts to promote faster wound healing, reduce swelling, and prevent complications.

Regular Follow-up Appointments

We schedule regular follow-up appointments with patients to assess recovery, care for wounds, remove sutures, or remove drains (if inserted), and address any questions or concerns that may arise. I'm ready to provide consultation and resolve any issues if patients have questions or concerns during the recovery period.

Access to Medical and Nursing Team

Patients can contact the clinic if they have any questions or experience unusual symptoms during recovery, to receive prompt care and advice.

I, Dr. Apiruk Wongsaowasup, or Dr. Teng, reiterate that at Aurora Clinic in Surat Thani and Koh Samui, "safety is our heart." We adhere to this philosophy strictly in every step to ensure that every patient who chooses us for their beauty needs receives beautiful, natural-looking results, and most importantly, utmost safety and peace of mind throughout their journey to renewed youth and confidence.

Dr. Teng, Dr. Apiruk Wongsaowasup, invited as a speaker on facial lifting with VXIL threads.

I, Dr. Apiruk Wongsaowasup, or Dr. Teng, of Aurora Clinic in Surat Thani and Koh Samui, am immensely proud to be a part of every patient's journey to discover beauty and confidence. I've witnessed the bright smiles, the faces that have regained their youth, and the happiness radiating from within countless patients after their "wrinkled, sagging face" and "crepey neck" problems were addressed precisely and artistically.

A Facelift is not just surgery; it's the "creation of personalized beauty," an investment in feeling good about yourself, and the restoration of "confidence" that shines from within. A naturally more youthful face will help you feel refreshed, energized, and ready to face every opportunity and challenge in life with full vigor.

If you are facing concerns about changes in your face and neck, whether it's sagging, deep wrinkles, or a crepey neck, please do not hesitate to consult me and the team at Aurora Clinic. We are ready to listen, understand, and offer the best options for you, so you can return to being your best self, with a youthful, radiant face, and brimming with confidence once again.

I sincerely hope that this path to a youthful face and lasting confidence will be a joyful and safe journey that you can embark on with us at Aurora Clinic. I look forward to meeting you all and being a part of creating confidence in your lives.
